WHAT IS HOSPICE?
It is funded by Medicare Part A and provides care for individuals with terminal diagnoses and their families. Hospice focuses on comfort, not curing the illness.
HOW DOES HOSPICE WORK?
Your doctor and the hospice team will collaborate with you and your loved ones to create a personalized care plan that caters to your specific needs.
Your plan of care will include a range of services, such as:
A dedicated hospice doctor
Compassionate nurses
Home health aides and homemakers
Supportive social worker
Understanding bereavement counselors and spiritual chaplains
Therapy and Nutrition Specialists (PT/OT/ST/Dietician)
Kind-hearted volunteers
Your doctor and the hospice team will collaborate with you and your loved ones to create a personalized care plan that caters to your specific needs.
FOUR LEVELS OF CARE
The Hospice Team will assess when a change in care level is necessary and determine the most suitable option for the patient.

Routine Home Care:
Provided when the patient is at home or in a Skilled Nursing Facility.

Acute Inpatient Care:
Offered during crisis periods in a Hospice Inpatient Unit or Skilled Nursing Facility. 3. Continuous Care: Given

Continuous Care:
Given during crises at home while acute symptoms are being managed.

Respite Care:
Available in a Skilled Nursing Facility if the primary caregiver needs a break for a 5-day period, occasionally.
Frequently Asked Questions
WHERE IS HOSPICE?
Hospice care is designed for individuals who have received a prognosis of having approximately six months or less to live if their illness progresses as expected.
HOW LONG CAN YOU RECEIVE HOSPICE?
Hospice services are typically available for up to six months before the expected time of passing. However, Medicare offers flexibility because the exact date of death is uncertain. If a patient exceeds the six-month timeframe, they can continue to receive Hospice Care as long as their Hospice Doctor verifies they are still terminally ill
BENEFIT PERIODS
In hospice care, you get care in benefit periods.
You can have two 90-day periods initially, then unlimited 60-day periods. Each starts when you start care and ends when the period finishes.
Before a new period, a doctor needs to recertify you are terminally ill and eligible for care. This ensures you get necessary
STOPPING HOSPICE
You are always in control of your hospice care journey. Feel free to pause or end hospice care whenever you feel it is necessary. And remember, if you meet the criteria, you can always return to hospice care whenever you need. Your comfort and well-being are our top priority.

WHAT MEDICARE WON'T COVER
- Treatment aimed at curing your terminal
- Prescription drugs meant to cure your illness instead of managing symptoms or relieving pain
- Emergency room visits, hospital stays, or ambulance rides, unless your hospice team arranges it or it’s not related to your terminal illness
